To critically examine under-five child caring practices among tobacco industry worker mother and mothers of nearby community. The present study was a purposively cross-sectional type. Sample were purposively selected from those family, which had at least one under-five children. Total sample was (n=150) which were classified into two characteristics primary caregiver (mother) worked as a tobacco worker and another segment was primary care giver (mother) who did not work in tobacco industry. There is a significant relationship between working and nearby community mothers for most of the studied variables. There were also observed that due to lack of caring practices children were malnourished more than nearby community mothers. P value for exclusive breast feeding, MUAC, mother’s occupation, weight for height, height for age were less than 0.05 which were considered statistically significant. Under-five children’s are a vulnerable segment of our society; their nutritional status is a strong indicator for health and therefore overall health situation of a community. Working class mothers were busy due to their occupation, most of the cases their children’s were cared by secondary caregiver which may cause malnutrition among children others factors were also responsible which were dig out through this study they were lack of caring knowledge, Illiteracy, low socio-economic status.

1. To critically examine under-five child caring practices among tobacco industry worker mother and mothers of nearby community.

Study location/area: Study was conducted randomly-selected 5 villages of kushtia district under sadar upazilla, a district with high tobacco production and processing industries. Study population: 150 families were selected randomly which had at least one 6-59 month children; families were categorized into two classes primary care giver of children(mother) working in a tobacco industry worker and children’s from nearby community whose mothers were not worked in tobacco industry. Study Design: The study was cross sectional as it was conducted at one point of time among the selected population. Sample size: Total sample were 150 randomly selected under-five children. Study period: present study was conducted over a six month period during 15 June, 2011 to 31 December, 2011. Development of questionnaire: A questionnaire was prepared for the purpose of the study. Then questionnaires were checked and crosschecked in order to correct inconsistency in information. Mothers of the selected children were interviewed to obtain information on their socioeconomic, demographic and health and nutrition-related issues included: age and sex of child, birth-order, number of total and under-five siblings, feeding and immunization history, type of residence and latrine, marital status of mother, monthly family income, and parental age, education, and occupation. Collection of data: The research assistant measured the children’s nude weight, using a frequently-standardized digital scale with 10 precision (Seca, model-345, Hamburg, Germany)for children aged less than two years, weight was calculated by subtracting the weight of the mother from the combined weight of the mother-child pair and height or length can be measured with a plastic tape; it is used for measuring stunting which is age-dependent and also in the cumulative effect of chronic malnutrition. The same research assistant also measured the weight sand heights of mothers, using standard procedures9.All measurements were taken thrice and averaged for the final reading. The study supervisor checked data forms daily. However, mid-upper arm circumference (MUAC) is a useful tool for a fast assessment of nutritional status of the child. MUAC provides an index of muscle wastage in under nutrition. MUAC was measured to the nearest millimeter at the exact midpoint of the left arm using a narrow, flexible, and non-stretchable tape made of plastic. Data processing and Data analysis: Information from finally checked questionnaires were entered in a data entry form using Statistical Package for Social Science (SPSS) Windows version 16 software and data entry were carefully done to avoid errors. Before data analysis, data analysis plan was formulated and dummy tables were drawn to measure the improvements of two comparative group. The data were analyzed by applying percentages, crosstabs, chi-square test, and correlation test. Descriptive analysis was undertaken to investigate dissimilarity of household information, socioeconomic information, caring practices, treatment seeking behavior morbidity pattern and clinical findings of children and anthropometric status of both worker and nearby community Chi-square test was used to test differences between categorical variables, p value less than 0.05 were considered as statistically significant for all tests of association. The results were demonstrated mostly in tabular forms and explicating graphical illustrations.

Poverty is one of the underlying causes of child malnutrition in our country, but it is not only one cause for child morbidity and mortality. The study gives us the picture of the severity of malnutrition among the children of tobacco workers which has significantly higher value than that obtained from nearby community. This study also focuses on some other contributing factors which may adversely affect child nutrition like maternal education and health, early marriage, child caring knowledge housing, sanitation etc. Caring is one of the basic needs for under-five children. Good Hygienic practices at home, work environment and opportunity for child caring may improve present scenario.
